Make sure you choose the form(s) from the correct academic year (the year is listed in the left-hand column of the chart below! The 2009-1010 forms are for Fall 2009/Spring 2010/Summer 2010 and the 2010-2011 forms are for Fall 2010/Spring 2011/Summer 2011.

Governor's (BOG) Fee Waiver Application
Use this form to apply for the BOG fee waiver for Fall 2010, Spring 2011 and/or Summer 2011. If you complete the FAFSA application, you are already applying for the BOG fee waiver with that application.

2010-2011 FA Information Form
This form is required for all financial aid applicants after their FAFSA has been received by CR. Students will be notified when this form is required. Be sure you need this form before printing!

2010-2011 Verification Worksheet - Independent This form is for INDEPENDENT (based on the FAFSA definition) students who have been selected for the verification process after completing the FAFSA application. Students will be notified when this form is required. Be sure you need this form before printing!

2010-2011 Verification Worksheet - Dependent This form is for DEPENDENT (based on the FAFSA definition) students who have been selected for the verification process after completing the FAFSA application. Students will be notified when this form is required. Be sure you need this form before printing!

2010-2011 Ability to Benefit Form
This form is required after a FAFSA has been completed if there is a question regarding whether a student has met the Ability to Benefit Requirement of HS Graduation or equivalent. Students will be notified if this form is required. Be sure you need this form before printing!

2010-2011 Academic Program Form This form may be required after a FAFSA has been completed to ensure that a student has chosen an eligible academic program before they receive financial aid. Students will be notified if this form is required. Be sure you need this form before printing!

2010-2011 Selective Service Form This form may be required after a FAFSA has been completed if a student is male and has not registered for selective service. Students will be notified if this form is required. Be sure you need this form before printing!

2010-2011 Status Information Letter Request Form This form may be required after a FAFSA has been completed if a student is male and has not registered for selective service AND is over 25 years of age. Students will be notified if this form is required. Be sure you need this form before printing!

2010-2011 Loan Process AND Application
NEW Direct Loan Process!!! The loan process explains the steps necessary before submitting the attached loan application and also provides information you should know before making a decision about a student loan. Read the loan process carefully! Students must first complete the FAFSA and online loan counseling before a loan request can be processed.

2010-2011 Appeal - FA Suspension This form may be required if a student is not meeting the Satisfactory Academic Progress (SAP) requirements (due to cumulative GPA below 2.0 and/or cumulative percent of units completed below 67%) and their financial aid has been suspended. SAP status is re-evaluated at the end of each semester, when grades are available. Students will be notified when this form is required. Be sure you need this form before printing!

2010-2011 Appeal - Extension of Maximum Timeframe This form may be required if a student is not meeting the Satisfactory Academic Progress (SAP) requirements (due to an excessive number of cumulative units attempted) and their financial aid has been suspended. SAP status is re-evaluated at the end of each semester, when grades are available. Students will be notified when this form is required. Be sure you need this form before printing!

2010-2011 ACG Rigorous Course of Study Verification This optional form can be used to document the completion of a rigorous program in High School, which is one of the requirements for the Academic Competitiveness Grant (ACG). The FAFSA application should be completed prior to submitting this form.

2010-2011 Special Conditions This optional form can be used to request a recalculation of your eligibility for financial aid based on changes in your financial circumstances. The FAFSA application must be completed and received by CR before this form is submitted to CR.

2010-2011 Review of Dependency Status Request This optional form can be used to document extenuating circumstances regarding a student's dependency status.

2010-2011 Dependency Status Update Form This optional form can be used to update Dependency Status AFTER a Dependency Status override was approved for a previous year.
